Home Health Aide

Job Summary

Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
  • Monitor and observe patients’ health status, recording vital signs such as blood pressure, pulse, and temperature accurately in electronic health record (EHR) or electronic medical record (EMR) systems
  • Support medication administration following proper procedures and documentation standards in compliance with HIPAA regulations
  • Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies
  • Assist with medical procedures such as catheter care, wound care, and Hoyer lift transfers to ensure safety and comfort
  • Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation
  • Perform light housekeeping tasks like cleaning, laundry, and maintaining a safe environment to promote overall well-being
  • Collaborate with healthcare teams to update medical records and ensure continuity of care in home & community settings

Skills

  • Strong patient observation skills to detect changes in health status promptly
  • Experience working in assisted living facilities, nursing homes, or long-term care environments
  • Proficiency with EMR/EHR systems for accurate medical documentation and record keeping
  • Knowledge of medical terminology relevant to home health care practices
  • Ability to assist with ADLs efficiently while maintaining patient dignity and independence
  • Certification in CPR and first aid; experience with hospice or dementia care is a plus
  • Familiarity with medical equipment such as Hoyer lifts, vital signs monitors, and mobility aids like wheelchairs or walkers
