Home Health Aide

Job Summary
Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. As a Home Health Aide, you will work closely with clients and their families to promote independence and comfort, delivering high-quality care in a supportive environment.

Duties

  • Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
  • Monitor and observe patient conditions, including vital signs, medication adherence, and overall health status
  • Provide medication administration following prescribed protocols while maintaining accurate medical documentation
  • Support patients with memory care needs, including Alzheimer’s and dementia care, ensuring a safe and comforting environment
  • Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a clean living space to promote hygiene and comfort
  • Document patient information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations
  • Collaborate with healthcare professionals to coordinate care plans and report any changes in patient condition promptly

Qualifications

  • Proven experience as a Home Health Aide or in similar caregiving roles within assisted living facilities, nursing homes, or home care settings
  • Knowledge of medical terminology and procedures related to patient care and medical documentation
  • Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or catheter care are a plus
  • Experience working with individuals with developmental disabilities or special needs preferred
  • Ability to operate medical equipment like Hoyer lifts and monitor vital signs accurately
  • Strong communication skills to interact effectively with patients, families, and healthcare teams
  • Compassionate attitude with a commitment to providing respectful and dignified care
