Home Health Aide

Duties

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and wheelchairs.
  • Monitor and observe patient health by taking vital signs, recording medical information accurately in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly.
  • Administer medications following prescribed schedules, ensuring compliance with HIPAA regulations and medical documentation standards.
  • Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques and patience.
  • Support patients with catheter care, wound dressing, hospice needs, and other complex medical procedures under supervision or according to training.
  • Prepare nutritious meals aligned with dietary restrictions and assist with feeding when necessary; perform light cleaning and laundry to maintain a safe and hygienic environment.
  • Document all patient interactions thoroughly in medical records, adhering to privacy standards while coordinating seamlessly with healthcare teams.

Experience

  • Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred.
  • Demonstrated ability to work effectively with individuals with developmental disabilities or disabilities in general.
  • Knowledge of medical terminology, CPR certification, and familiarity with EMR/EHR systems for accurate documentation.
  • Experience providing dementia or Alzheimer’s care within a home or community setting is advantageous.
  • Long-term care experience that highlights your commitment to patient-centered caregiving and safety protocols.
  • Proven ability to handle emergency situations calmly while executing tasks such as vital sign monitoring or catheter care confidently.
