Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts when necessary
- Monitor and observe patients’ conditions regularly, documenting vital signs and changes in health status accurately within Electronic Health Record (EHR) systems
- Medication reminders and manage treatments following prescribed medical protocols while maintaining strict adherence to HIPAA regulations for patient privacy
- Support clients with specialized needs such as dementia, Alzheimer’s disease, or developmental disabilities through tailored memory care and behavioral strategies
- Prepare nutritious meals and assist with feeding, ensuring dietary restrictions are followed precisely
- Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment to promote overall health and hygiene
- Document medical records meticulously, including medication administration logs and care notes to ensure seamless communication among healthcare providers
Qualifications
- Proven experience in home health care, long-term care facilities, nursing homes or assisted living environments
- Knowledge of medical terminology, vital signs measurement, catheter care, hospice care practices, and CPR certification preferred
- Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation
- Ability to work with individuals with disabilities or cognitive impairments such as dementia or Alzheimer’s disease
- Strong patient observation skills coupled with compassionate caregiving abilities
- Experience operating mobility aids like walkers and working with medical equipment such as catheters or medication dispensers is highly desirable
- Excellent communication skills to collaborate effectively with clients, families, and healthcare teams
