Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and wheelchairs.
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status.
- Administer medications accurately following medical instructions and document all medical records in Electronic Health Record (EHR) systems while maintaining HIPAA compliance.
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory-related conditions through tailored memory care techniques.
- Support patients with medical procedures such as catheter care, wound dressing, and hospice care needs under supervision or as trained.
- Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical guidelines.
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being.
- Document patient care activities thoroughly using medical documentation standards and electronic systems to ensure accurate records are maintained for long-term care planning.
- Collaborate effectively with healthcare teams by working with EMR/EHR systems to update patient information promptly and accurately.
- Provide emotional support and companionship to clients, fostering a positive environment that promotes independence and dignity.
Experience
- Prior experience in assisted living facilities, nursing homes, or home health environments is highly desirable.
- Demonstrated ability to work with individuals with developmental disabilities or those requiring specialized dementia or Alzheimer’s care.
- Knowledge of medical terminology and familiarity with medical documentation processes.
- Certification in CPR is required; additional certifications such as medication administration, hospice care, or catheter care are preferred.
- Experience working with electronic health records (EHR/EHR systems) and understanding HIPAA regulations is essential.
- Long-term care experience or home & community care background will enhance your ability to deliver exceptional patient-centered services.
- Ability to handle physical tasks involving lifting or transferring patients safely using equipment like Hoyer lifts is necessary.
