Join our dedicated team as a Home Health Aide & Caregiver, where you will play a vital role in enhancing the quality of life for individuals in their homes and community settings. This rewarding position offers the opportunity to provide compassionate, personalized care to diverse populations, including seniors, individuals with disabilities, and those recovering from illness or injury. As a key member of our caregiving team, you will deliver essential support that promotes independence, dignity, and well-being.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ health status, recording vital signs and changes in condition accurately
• Administer medications and manage medical documentation in compliance with HIPAA regulations
• Support clients with specialized needs such as Alzheimer’s care, dementia care, hospice care, and memory care
• Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities and medical records
• Use assistive devices like Hoyer lifts safely to transfer clients and prevent injury
• Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment at home or in assisted living facilities

Skills

• Strong patient observation skills to identify changes in health or behavior promptly
• Experience working with individuals with developmental disabilities or in long-term care environments
• Knowledge of medical terminology and procedures related to catheter care, medication administration, and vital signs measurement
• Ability to work effectively with elderly populations and those requiring memory or dementia care
• Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
• CPR certification and knowledge of emergency response protocols
• Compassionate caregiving approach combined with excellent communication skills to build trust with clients and families
