As a Certified Home Health Aide (CHHA), you will play a critical role in supporting patients with personal care needs, assisting with activities of daily living, and promoting safety, comfort, and quality of life in the home setting.

Requirements

• Active, unrestricted California Home Health Aide (CHHA) Certification (minimum 1 year required)
• Minimum 1 year of Home Health experience Required
• Current CPR certification
• Current TB clearance
• Current Health Screening/Physical
• Reliable transportation and valid California driver’s license & Insurance
• Proficiency in electronic documentation (WellSky experience preferred)
• Strong observation skills and ability to report changes in condition
• Ability to work independently and manage assignments effectively
• Professional, compassionate, and patient-centered approach

Responsibilities

Under the direction of the RN and in accordance with the Plan of Care, the CHHA will:

• Perform patient care activities and ongoing observations
• Monitor and report changes in patient condition and vital signs
• Provide supportive care services including:
• Personal care (bathing, grooming, dressing)
• Toileting and incontinence care
• Ambulation and transfer assistance
• Range of motion exercises
• Meal preparation and feeding assistance
• Light housekeeping related to patient care
• Provide assistance with medication reminders (no administration)
• Educate patients and caregivers on safety and basic care needs
• Identify and promptly report changes in condition to the RN and office team
• Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes

Pay: $45.00 - $50.00 per hour
