Job Description:

Quality Health Care Group, a leading provider of home and community-based services, is seeking compassionate and dedicated Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), and Direct Care Workers to join our team. We specialize in serving individuals with intellectual disabilities, developmental disabilities, and those receiving services under the Office of Developmental Programs (ODP). Experience working with individuals in this patient population or with Traumatic Brain Injury (TBI) is a strong plus.

You will play a key role in helping our clients maintain their independence and quality of life in a home or community setting.

Responsibilities:

• Assist clients with daily living activities including bathing, dressing, grooming, and toileting

• Provide mobility support and assistance with transfers

• Provide mobility support for patient appointments

• Prepare meals and assist with feeding, as needed

• Perform light housekeeping duties to maintain a clean and safe environment

• Engage clients in recreational or community-based activities

• Monitor and report changes in client behavior or health to the care team

• Maintain accurate documentation and follow care plans

• Follow all agency policies and ODP guidelines

Qualifications:

• Experience with Therap/Axxess strongly preferred

• CNA Certification, HHA Certification, or at least 4 years of relevant direct care experience

• High school diploma or GED required

• Experience with ODP programs a strong plus

• Experience with TBI clients strongly preferred

• Ability to work independently and follow instructions

• Reliable transportation and valid driver’s license preferred

• Must pass background check and required health screenings
