Position Available: Full Time Intake Coordinator
Job Duties and Responsibilities:
- Serve as the primary point of contact for patients and their families, coordinating intake processes and scheduling initial appointments.
- Conduct comprehensive assessments of patients' needs and eligibility for home health and hospice services.
- Coordinate with healthcare providers, caregivers, and interdisciplinary team members to ensure seamless transitions of care for patients.
- Schedule home visits, therapy sessions, and other appointments based on patients' needs and availability.
- Maintain accurate and up-to-date patient records, including demographic information, insurance verification, and authorization documentation.
- Communicate effectively with patients, families, and external stakeholders to provide information, support, and assistance as needed.
- Collaborate with billing and administrative staff to ensure timely and accurate processing of referrals and authorizations.

Qualifications:
- Bachelor's degree in healthcare administration, nursing, or related field preferred.
- Previous experience in intake coordination, scheduling, or healthcare administration is advantageous.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare professionals.
- Proficiency in electronic health records (EHR) systems and Microsoft Office suite.
- Compassionate and empathetic attitude towards patients and their families.
- Ability to work independently and as part of a collaborative team.
