Job Summary
We are seeking a dynamic and compassionate Geriatric Nurse Practitioner (NP) specialized in Home Health Care to join our dedicated team. Responsibilities
- Conduct thorough assessments of elderly patients in their home environments, including vital signs, physical examinations, and functional evaluations.
- Develop and implement individualized care plans that incorporate medication management, chronic disease management, and preventive health strategies.
- Manage acute and chronic conditions such as hypertension, diabetes, dementia, and mobility issues through evidence-based interventions.
- Coordinate with multidisciplinary teams to ensure seamless care transitions, discharge planning, and post-acute care follow-up.
- Administer treatments including IV infusions, injections (e.g., botulinum toxin treatment), wound suturing, and catheter care while maintaining sterile techniques.
- Utilize electronic health record (EHR) systems like Epic or eClinicalWorks for accurate documentation of patient encounters and medical records.
- Provide health coaching and education tailored to individual patient needs, empowering them to manage their health effectively at home.
- Participate in telehealth consultations and triage services to address urgent concerns promptly and efficiently.
- Ensure compliance with HIPAA regulations and maintain the highest standards of infection control and patient safety throughout all clinical activities.
Requirements
- Valid Nurse Practitioner (NP) license with certification in geriatrics or family practice.
- Proven experience working with elderly populations in home health or outpatient settings.
- Strong knowledge of medical terminology, physiology, diagnostic evaluation, and medication administration.
- Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation purposes.
- Experience with complex medical procedures including IV insertion, venipuncture, catheterization, and sterile processing techniques.
- Critical care experience or hospital-based practice in settings like ICU or Level I Trauma Centers is highly desirable.
- Knowledge of CPT coding for billing purposes along with familiarity with Workers' Compensation law where applicable.
- Ability to perform comprehensive patient assessments including vital signs, physical exams, and functional evaluations.
- Excellent communication skills for effective collaboration with patients’ families and healthcare teams.
