Job Summary
We are seeking a dynamic and compassionate Geriatric Nurse Practitioner (NP) specialized in Home Health Care to join our dedicated team. Responsibilities

• Conduct thorough assessments of elderly patients in their home environments, including vital signs, physical examinations, and functional evaluations.
• Develop and implement individualized care plans that incorporate medication management, chronic disease management, and preventive health strategies.
• Manage acute and chronic conditions such as hypertension, diabetes, dementia, and mobility issues through evidence-based interventions.
• Coordinate with multidisciplinary teams to ensure seamless care transitions, discharge planning, and post-acute care follow-up.
• Administer treatments including IV infusions, injections (e.g., botulinum toxin treatment), wound suturing, and catheter care while maintaining sterile techniques.
• Utilize electronic health record (EHR) systems like Epic or eClinicalWorks for accurate documentation of patient encounters and medical records.
• Provide health coaching and education tailored to individual patient needs, empowering them to manage their health effectively at home.
• Participate in telehealth consultations and triage services to address urgent concerns promptly and efficiently.
• Ensure compliance with HIPAA regulations and maintain the highest standards of infection control and patient safety throughout all clinical activities.

Requirements

• Valid Nurse Practitioner (NP) license with certification in geriatrics or family practice.
• Proven experience working with elderly populations in home health or outpatient settings.
• Strong knowledge of medical terminology, physiology, diagnostic evaluation, and medication administration.
• Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation purposes.
• Experience with complex medical procedures including IV insertion, venipuncture, catheterization, and sterile processing techniques.
• Critical care experience or hospital-based practice in settings like ICU or Level I Trauma Centers is highly desirable.
• Knowledge of CPT coding for billing purposes along with familiarity with Workers' Compensation law where applicable.
• Ability to perform comprehensive patient assessments including vital signs, physical exams, and functional evaluations.
• Excellent communication skills for effective collaboration with patients’ families and healthcare teams.
