Job Overview
We are seeking a dynamic and dedicated Home Health Care Liaison to serve as the vital link between healthcare providers, patients, and home health agencies. Responsibilities
- Develop and maintain strong relationships with physicians, hospitals, skilled nursing facilities, clinics, case managers, and community healthcare partners
- Generate new patient referrals and meet or exceed monthly admission goals
- Conduct engaging product demonstrations and presentations to educate providers about available home health solutions.
- Facilitate smooth onboarding processes for new patients and coordinate care plans in collaboration with medical teams.
- Negotiate service agreements and contracts with healthcare facilities to expand referral networks.
- Provide exceptional customer service by addressing provider inquiries promptly and professionally.
- Analyze referral patterns and market trends to identify opportunities for growth and service improvement.
- Organize and deliver professional sales presentations using Microsoft PowerPoint, emphasizing the benefits of home health care programs.
Requirements
- Minimum 2 years of recent experience as a Home Health Liaison or in home‑health‑specific marketing/referral development
- Strong knowledge of Medicare guidelines and home health eligibility is a plus
- Established referral network is highly desirable
- Strong negotiation skills coupled with excellent account management capabilities.
- Demonstrated ability to conduct product demos and technical sales effectively.
- Exceptional communication skills, both verbal and written, with an emphasis on active listening and clear articulation of complex information.
- Strong organizational skills to manage multiple accounts and coordinate care seamlessly.
- Analysis skills to interpret market data, referral trends, and customer feedback for strategic planning.
- Proficiency in Microsoft Office Suite, including Microsoft Word for documentation and Microsoft PowerPoint for presentations.

Pay: $60,000.00 - $100,000.00 per year
