Home Health Caregiver -

We are looking for a compassionate CNA/Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan. In addition, we are looking for Caregivers with transfer, hoyer lift experience. Companion Caregivers are welcome to apply as well. Language skills needed : Mandarin, Korean, etc..

Responsibilities

• Provide health care services in patients residences
• Perform domestic and household tasks
• Transport and accompany patients to doctors office or to hospital
• Administer simple prescribed medication/reminders
• Assist with clients personal care activities(Activities of Daily Living)
• Monitor patients (vital signs, temperature, respiration, etc) and report on their condition
• Maintain patients care records and document provided services
• Assist patients with mobility and physical therapies/exercises
• Instruct and counsel patients and families on diet and exercise
• Collect routine specimens
• Provide companionship and basic emotional or psychological support- do activities with client - play cards, memory games, etc.
• Log in and out of Wellsky or other systems used to record service hours
• Drive to doctor appointments, and other wellness activities

Skills

• Personal /Companion care aide
• Proven working experience as home health aide/CNA - Activities of Daily Living(ADLs)
• Ability to monitor vital signs and to collect information and report as needed
• Familiarity with basic nutrition and personal hygiene standards
• Proficiency in English, and other languages as needed
• Nursing and health care administration skills - provide updates on care plan, client changes
• Caring and compassionate personality
• Current CPR certificate
• First Aid training
• Valid driver’s license

Job Types: Full-time, Part-time

Pay: $17.00 - $22.00 per hour
