Home Health Case Manager

Position Overview

The Home Health Case Manager plays a critical role in coordinating and managing patient care within the home setting under an In-Home Care Provider (IHCP) license. This position is responsible for developing, implementing, and monitoring individualized care plans to ensure patients receive high-quality, compliant, and compassionate care.

The Case Manager serves as the primary liaison between patients, families, caregivers, and healthcare providers—ensuring services are delivered efficiently, safely, and in accordance with Department of Health regulations.

Key Responsibilities

  • Conduct initial and ongoing patient assessments to determine care needs
  • Develop, implement, and update individualized care plans
  • Coordinate services between patients, caregivers, and healthcare providers
  • Monitor patient progress and adjust care plans as needed
  • Maintain accurate and timely documentation in compliance with IHCP and Department of Health standards
  • Ensure all services meet state, federal, and agency regulatory requirements
  • Communicate regularly with patients and families regarding care plans and progress
  • Support scheduling and oversight of caregiver assignments
  • Identify risks, changes in condition, or gaps in care and escalate appropriately
  • Participate in team meetings and contribute to continuous improvement of care delivery

Qualifications

  • High school diploma or equivalent required; Associate’s degree or higher preferred
  • Experience in case management, home health, social work, or healthcare coordination preferred
  • Basic medical knowledge and familiarity with medical terminology preferred
  • Understanding of home health regulations and IHCP operational standards preferred
  • Completion and passing of a Department of Health-required background check
  • Valid driver’s license, current auto insurance, and reliable transportation for patient visits
  • Ability to comply with all state and federal regulations governing in-home care services
  • Demonstrated ability to maintain confidentiality in accordance with HIPAA guidelines
  • Ability to pass any required health screenings, drug screenings, or competency evaluations as required by state regulations

Work Environment

  • Combination of office-based coordination and field-based patient visits
  • Regular travel to patient homes within the service area
  • Flexible scheduling based on patient needs

Pay: $20.00 - $32.00 per hour
