Home Health Case Manager (RCD Experience Required)

We are seeking an experienced Home Health Case Manager to oversee patient care, coordinate services, and ensure compliance with Medicare guidelines, including Review Choice Demonstration (RCD) submissions. The ideal candidate is organized, detail-oriented, and confident managing documentation and patient care plans.

Key Responsibilities:

  • Manage and coordinate care for a caseload of home health patients
  • Complete and submit accurate and timely RCD documentation
  • Ensure compliance with Medicare, state, and company guidelines
  • Collaborate with physicians, therapists, and interdisciplinary team members
  • Review OASIS documentation and care plans for accuracy
  • Provide patient and caregiver education as needed

Qualifications:

  • Active RN license (required)
  • Home health experience (required)
  • Experience with RCD submissions and Medicare documentation (required)
  • Strong organizational and time management skills
  • Knowledge of OASIS and home health regulations
