Home Health Case Manager/Community Liaison (RN Required)

Job Summary
Abundant Life Home Health Agency is seeking a dynamic and clinically knowledgeable Home Health Case Manager / Community Liaison to support patient care coordination and referral development. This dual-role position combines clinical case management oversight with community outreach and relationship building to ensure high-quality, compliant, and patient-centered home health services.

The ideal candidate is experienced in home health operations, understands Medicare/Medicaid regulations (42 CFR §484), and excels in both clinical coordination and business development.

Responsibilities

Clinical Case Management

  • Conduct and/or oversee initial and ongoing patient assessments, ensuring compliance with physician orders and Plan of Care (POC)
  • Coordinate interdisciplinary services to meet patient needs in accordance with Medicare Conditions of Participation
  • Ensure timely recertifications, reassessments, and care plan updates
  • Monitor documentation for accuracy, completeness, and regulatory compliance (OASIS, visit notes, orders)
  • Collaborate with physicians, nurses, aides, and therapists to ensure continuity of care
  • Participate in case conferences and Quality Assurance/Performance Improvement (QAPI) activities
  • Ensure services are delivered in accordance with individualized POC and agency policies

Community Liaison / Business Development

  • Develop and maintain relationships with hospitals, physicians, rehab facilities, assisted living facilities, and community organizations
  • Educate referral sources on agency services, admission criteria, and patient eligibility
  • Generate and track referrals to support agency census growth
  • Represent the agency at community events, healthcare meetings, and networking opportunities
  • Conduct facility rounding and outreach visits
  • Maintain professional communication and follow-up with referral partners

Qualifications

  • Licensed RN in the State of Florida
  • Minimum 2-3 years home health experience (case management home health/hospital/insurance required)
  • Knowledge of:
  • Medicare/Medicaid home health regulations
  • OASIS documentation requirements
  • Plan of Care development and physician orders
  • Strong communication, organizational, and interpersonal skills
  • Ability to work independently in a field-based role
  • Valid driver’s license and reliable transportation

Preferred Qualifications

  • Experience with referral development or healthcare marketing
  • Familiarity with Joint Commission standards
  • Established relationships within local healthcare community (Tampa Bay area a plus)

Pay: $62,400.00 - $80,000.00 per year
