Home Health Clinical Coordinator (LVN/LPN)

Pay range: $38.00-$42.00 per hour, depending on experience
Schedule/Shift: Part-time working Monday-Friday, 10am-2pm
Territory/Location: Ventura County
Responsibilities:
  • Reviews clinical documentation to help determine home health appropriateness.
  • Assists in compiling and reporting on clinical metrics shared with community partners.
  • Tracks and updates team members on new hospitalizations, hospital holds and resumptions of care.
  • Must be self-driven with a high degree of attention to detail.
  • Communicates with patients, families and caregivers regarding home health services.
  • Coordinates with physicians and referral sources (as appropriate) regarding home health plans of care.
  • Assists in ensuring that patients are seen in a timely manner and works with physicians to receive updated start of care orders when necessary.
  • Cross trains to provide back-up coverage for scheduling and intake coordination when needed.
  • Assist with field visits if needed.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Other tasks and responsibilities as assigned.
Qualifications:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • One (1) year nursing experience in home care, hospice care, institutional hospital or facility care preferred.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of service.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
  • Possess and maintains current BLS CPR Certification.
