Home Health Clinical Scheduler/ Care Coordinator

Job Description Summary:

The Home Health Scheduler has the primary responsibility of scheduling field staff for ongoing visits with patients. There may be collaboration needed from the home health scheduler with the intake department for initial assignments. Computer-based applications are typically used to manage the scheduling system.

Essential Job Functions/Responsibilities:

  • The scheduler will have a documented daily forecast on each nurse and when they are available to see patients and in what territories. It is preferred to have these schedules submitted and documented two-weeks in advance unless there is an emergency. Updates and maintains computerized schedules/calendar on a regular basis throughout the day.
  • It is the scheduler?s responsibility to have outstanding communication skills to ensure timely scheduling based on the needs of the patient from the following clinicians: registered nurse, physical therapist, occupational therapist, speech therapist, medical social workers, home health aides, dieticians, etc.
  • The scheduler will plot all missed visits, transfers and track re-certifications.
  • The scheduler is responsible for scheduling the first start of care visit.
  • In collaboration with the Palliative Care Director/Clinical Manager, the scheduler will assign palliative care trained staff to patients identified as palliative care patients.
  • The scheduler is responsible to schedule the nursing resumption of care (ROC) Works with Intake to assist in providing referred clients with initial field staff availability.
  • Outside contracted Physical Therapy company will keep in contact with scheduler and communicate the scheduled duration of PT, OT, and ST visits.
  • Once scheduled with nursing, the scheduler will be responsible for organizing schedules to indicate needed follow-ups such as F2F and CMS 485.
  • The scheduler coordinates the scheduling of all uncovered visits for clinicians who are off, sick or unable to see due to full workload.
  • The scheduler effectively utilizes field staff in order to ensure full optimization. Taking into account location, patient?s needs and clinician case load, works toward managing individual patients? schedule with consistent staff and appropriate visit times.
  • The scheduler answers incoming scheduling phone calls from clinicians responding in a courteous and professional manner and uses appropriate interdepartmental communication to support essential agency activities.
  • The scheduler communicates promptly and appropriately with the field staff, patients, and family regarding scheduling needs and incoming calls.
  • The scheduler communicates to leadership team regarding additional staffing needs and may also perform other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Archer Health.
  • Confirms and verifies in-patient admissions and other outside services. Maintains good communication with patients and families during hospitalization either directly or in collaboration with appropriate personnel. (marketer, patient?s case manager, hospital discharge planner, etc.)
  • Ensures that patient's EMR is up to date with information on hospitalizations, ER visits and community organizations (such as Adult Day Care) related to their health.
  • Responsible for Durable Medical Equipment (DME) tracking and confirming that DME is delivered to patient.
  • Maintains ongoing tracking and appropriate documentation on any orders to promote team awareness and physician education. This tracking may use an IT database.
