Job Summary
We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality patient care in the comfort of their homes. The ideal candidate will possess strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team. This role involves delivering nursing care, educating patients and their families, and ensuring adherence to treatment plans.
Duties
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, including ostomy care and tube feeding.
- Monitor patient progress and report any changes in condition to the healthcare team.
- Maintain accurate medical records using eClinicalWorks software.
- Educate patients and families on disease management, medication administration, and home community care practices.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
- Provide support for outpatient procedures such as cardiac catheterization when necessary.
- Participate in quality improvement initiatives to enhance patient outcomes.
Experience
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in a clinic or teaching hospital setting is preferred.
- Familiarity with home health care practices and standards is highly desirable.
- Proficient in using electronic medical records systems, particularly eClinicalWorks.
- Experience with cardiac catheterization, ostomy care, and tube feeding is a plus.
- Strong communication skills and the ability to teach patients effectively.
- Ability to work independently while managing multiple priorities in a home health environment.
