Duties:

• Provide skilled nursing care to patients in their homes
• Perform comprehensive assessments of patients' health status
• Develop and implement individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor and evaluate patients' progress and update care plans as needed
• Educate patients and their families on managing their conditions and promoting wellness
• Coordinate with other healthcare professionals to ensure continuity of care
• Assist with discharge planning for patients transitioning from hospital to home
• Provide support and guidance to patients and their families in making healthcare decisions

Skills:

• Valid LPN/LVN license in the state of Colorado
• Minimum of 1 year of nursing experience, preferably in home health or a related field
• Strong clinical skills and knowledge of medical procedures and treatments
• Excellent communication and interpersonal skills
• Ability to work independently and make sound judgments
• Proficiency in electronic medical record systems, such as HomeCare HomeBase/Pointcare, is a plus

Pay: $32.00 - $42.00 per hour
