About the Role

As an LVN in home health, you will provide high-quality nursing care to patients in their homes, offering support, guidance, and empathy during every step of their health journey. Your role includes:

• Conducting home visits to deliver individualized patient care.
• Administering medications and managing treatments.
• Educating patients and families on health conditions and recovery plans.
• Collaborating with the care team to ensure continuity of care.

What We’re Looking For

• A licensed LVN passionate about home health care.
• Strong communication and interpersonal skills.
• A heart for patient advocacy and a commitment to excellence.
• Experience in home health or hospice (preferred but not required).
• Experience with Kinnser/WellSky (preferred but not required).

Job Type: Full-time

Pay: $30.00 - $35.00 per hour
