Qualifications and Requirements:

• Current license with the state Board of Nursing
• 1 year of nursing experience is required - home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Responsibilities

• Provide comprehensive patient care in home health settings, including checking vitals, medication management, teaching of patientand Family, catheterization, IV insertion, and tube feeding.
• Conduct thorough patient assessments and maintain accurate electronic health records (EHR).
• Administer basic life support and manage airway as needed.
• Collaborate with healthcare teams to develop and implement individualized care plans.
• Ensure compliance with safety protocols while assisting patients with mobility and heavy lifting tasks.

Maintain a valid RN license and a driver's license for travel to patient locations.

Pay Rate Per Visit For RN

SOC’s - $100

Follow Up Visit - $60

ROC - $80

Discharge -$80

High Tech Visit- $80

Pay Rate Per Visit For LPN

Regular Follow Up Visit- $55

Monthly Bonus For On Time Notes And Referrals!!!!!
