Home Health Nurse

Job Summary

Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.
  • Administer medications and injections with precision, including IV infusions and other advanced therapies, ensuring adherence to safety protocols.
  • Manage complex wound care, including dressing changes and infection control measures for patients with chronic or acute wounds.
  • Provide education on disease management, medication administration, nutrition, and self-care techniques tailored to individual needs.
  • Coordinate with interdisciplinary teams using EMR (Electronic Medical Records) systems such as Epic or Cerner to document patient progress accurately and efficiently.
  • Assist with discharge planning and transition of care from hospital or facility to home setting, ensuring seamless continuity of services.
  • Respond promptly to triage calls and urgent situations, utilizing critical thinking skills to prioritize interventions and escalate care when necessary.

Skills

  • Proven experience in critical care environments such as ICU or Level I Trauma Centers; familiarity with acute pain management and infectious disease care is highly valued.
  • Strong knowledge of EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks; proficient in ICD-9/10 coding for accurate documentation.
  • Expertise in managing diverse populations including geriatrics, pediatrics, individuals with developmental disabilities, dementia or Alzheimer’s disease, and those requiring hospice or palliative care.
  • Skilled in advanced nursing procedures such as IV insertion, catheterization, sterile technique, tube feeding management, and ventilator support.
  • Excellent patient assessment abilities combined with physiology knowledge for diagnostic evaluation and medication administration.
  • Ability to work independently in home & community settings while maintaining high standards of infection control and HIPAA compliance.
  • Effective communication skills for collaborating with families, caregivers, physicians, and other healthcare professionals across various medical disciplines including emergency medicine and outpatient services.
