Overview
We are seeking a dedicated Home Health Nurse to provide comprehensive nursing care to clients in their homes. The ideal candidate will possess a strong clinical background, including critical care experience and proficiency with electronic health record systems. This role offers an opportunity to make a meaningful difference by delivering high-quality patient-centered care across diverse settings.

Duties

• Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations.
• Administer medications, injections, and perform catheterizations.
• Manage complex cases involving acute pain, dementia, Alzheimer's disease, and infectious diseases while coordinating with interdisciplinary teams.
• Provide specialized care such as wound management, tube feeding, ventilator support, and post-acute care procedures.
• Document all patient interactions accurately using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems in compliance with HIPAA regulations.
• Educate patients and their families on health coaching, medication administration, and disease management strategies.
• Support patients with developmental disabilities or those requiring memory or behavioral health management in home and community settings.

Experience

• Proven nursing experience with a background in critical care environments.
• Demonstrated expertise in home health care, hospice, or post-acute settings with familiarity in ICD-9/ICD-10 coding for documentation accuracy.
• Experience working with diverse populations including geriatrics, individuals with disabilities, or those requiring dementia or Alzheimer's care.
• Knowledge of infection control protocols and sterile processing techniques essential for safe patient care delivery.
