Job Overview

The Home Health RN performs Skilled nursing assessments in client's homes. The role requires clinical expertise, instructional abilities, and knowledge of Florida Home Health Clinical operations. This is a Hybrid role with 90% in the field and 10% in the office.

Responsibilities

• Conducting OASIS patient assessments in compliance with regulatory criteria
• Coordinating patient care plans with physicians, hospitals, and other providers
• Communicating care protocols to patients/families to ensure compliance with prescribed treatment plan and instructions
• Monitoring regulatory information to ensure compliance with applicable healthcare regulations, standards, and policies
• Maintaining accurate medical records and documentation in accordance with legal and ethical standards
• Evaluating on-site skills competency for Home Health Aides

Requirements

• Current Florida RN license
• Ability to Pass Level 2 Background Check including fingerprinting
• 1-2 years nursing experience, preferably in home health settings
• Ability to travel to client homes within Volusia and Flagler County
• Ability to work independently with minimal direct supervision
• Strong ability to communicate with diverse groups of people
• Proficient with electronic health records and mobile communication systems
• Knowledge of HIPAA confidentiality and data protection
• Experience in home health clinical instruction preferred
• Knowledge of Florida AHCA regulations for home health agencies preferred

Pay: $40.00 - $45.00 per hour
