Join our dedicated healthcare team as a Home Health Nurse or Licensed Vocational Nurse (LVN) serving Houston and surrounding areas! Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories.
  • Administer medications via injections, IV infusions, and other routes while adhering to safety protocols such as HIPAA regulations and aseptic techniques.
  • Develop and implement individualized care plans tailored to each patient’s needs, including those with complex conditions like dementia or acute pain management.
  • Provide specialized care for patients with chronic illnesses such as Alzheimer's disease, memory care needs, or infectious diseases.
  • Coordinate discharge planning and collaborate with physicians and other healthcare professionals to ensure seamless continuity of care.
  • Educate patients and families on disease management, medication administration, nutrition, and health coaching strategies.
  • Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner; ensure compliance with medical documentation standards.

Experience

  • Proven experience in home health nursing or LVN roles with a strong foundation in patient care across various settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics.
  • Critical care experience including ICU or Level I Trauma Center exposure is highly desirable.
  • Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation procedures.
  • Hands-on experience with IV insertion, catheterization, tube feeding management, ventilator support, and infection control practices.
  • Knowledge of EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
  • Ability to perform triage assessments and work effectively with individuals with disabilities or developmental challenges.
  • Experience working in specialized areas such as hospice care, geriatrics, pediatrics—including neonatal ventilator management—and post-acute care environments.
