Home Health Nursing Specialist

Nightingale Care is seeking a compassionate and skilled nurse to join our team, with availability to work weekends as needed.

In this role, you will provide individualized, high-quality care to patients in the comfort of their homes. Responsibilities include assessing patient needs, developing and implementing care plans, administering medications, and educating patients and their families on effective health management.

You will collaborate closely with physicians, therapists, and other healthcare professionals to ensure optimal patient outcomes while building strong, supportive relationships with those in your care.

Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status (OASIS)
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans as needed
- Educate patients and their families on managing their health conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records

Qualifications:
- Valid RN license in the state of Ohio OR Valid LPN License in the state of Ohio
- Minimum of 1 years of nursing experience, preferably in home health
- Proficient in medical record systems and documentation
- Knowledge of aseptic technique and infection control practices
- Strong patient assessment skills
- Experience working with individuals with developmental disabilities is a plus
- Ability to work independently and prioritize tasks effectively
- Excellent communication and interpersonal skills

Pay: $30.00 - $45.00 per hour
