Position Summary

We are seeking a skilled and compassionate Physical Therapist (PT) to join our growing home health team. The PT/PTA will provide high-quality rehabilitative care to patients in their homes, develop individualized treatment plans, and work collaboratively with physicians and the interdisciplinary care team to ensure positive outcomes.

Responsibilities

• Conduct initial evaluations and develop individualized Plans of Care.
• Provide physical therapy services as ordered by the physician and in accordance with agency policies.
• Teach patients and caregivers safe and effective ways to improve mobility, strength, and function.
• Reassess and update the Plan of Care as needed, including after hospitalizations or condition changes.
• Document all patient care accurately and in a timely manner per agency and state standards.
• Communicate effectively with physicians, RNs, LVNs, and office staff regarding patient status and progress.
• Participate in interdisciplinary case conferences and quality improvement initiatives.
• Supervise and provide guidance to PTAs as required.
• Promote safety, independence, and quality of life for patients.
• Maintain professional growth and participate in ongoing education and training.

Qualifications

Required:

• Current California Physical Therapist
• Valid Driver’s License and reliable vehicle with insurance
• Ability to travel to patient homes across Los Angeles County
• Strong interpersonal and communication skills
• Ability to lift, transfer, and assist patients as needed

Preferred:

• Experience in home health, hospital, or outpatient rehabilitation settings
• Bilingual communication skills (any language)
• Familiarity with electronic medical records (EMR)

• California Physical Therapist Proof of License
• Driver’s License & Auto Insurance
• TB Test / Chest X-Ray
• Physical Exam
• CPR Certification

Job Types: Full-time, Per diem

Pay: $110.00 - $145.00 per hour
