Duties

  • Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments
  • Establish a treatment plan in consultation with the physician and revises with the approval of the physician as necessary
  • Assists in development and implementation of the interdisciplinary patient care plan as it pertains to physical therapy
  • Consults and familiarizes staff regarding the established plan
  • Communicates with the physician and the rest of the care team regarding the patient's status and obtains additional orders, if necessary

Experience

  • Proven experience in physiotherapy within home health settings or outpatient clinics is highly preferred.
  • Strong background in acute care hospital environments with exposure to inpatient rehabilitation or orthopedic units enhances your ability to manage diverse patient needs effectively.
  • Familiarity with home & community care practices and outpatient therapy techniques is essential for delivering comprehensive patient-centered services.
  • Knowledge of anatomy and physiology fundamentals underpins effective assessment and treatment planning across various patient populations.
  • Hands-on experience with EMR systems ensures efficient documentation and seamless communication within healthcare teams.
  • Prior work involving pediatrics or geriatrics demonstrates your versatility in caring for vulnerable populations requiring specialized approaches.
  • Additional skills such as trigger point therapy or orthopedics expertise will enable you to provide targeted interventions that accelerate recovery. Pay: From $60.00 per hour

Expected hours: No less than 10 per week
