Responsibilities

• Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop effective care plans.
• Deliver skilled therapy interventions for the geriatric population with a variety of diagnoses.
• May perform Oasis Start of Care Assessments.
• Coordinate discharge planning with healthcare team.
• Implement and adjust individualized care plans based on patient progress and changing needs, utilizing EMR systems for documentation.
• Educate patients and their families on exercises, safety measures, and health maintenance strategies to foster independence.
• Monitor patient progress regularly, documenting outcomes and modifying treatment approaches as necessary.
• Collaborate with multidisciplinary teams including physicians, nurses, and social workers to optimize patient care in the home and community settings.

Skills

• Strong knowledge of physiology and anatomy relevant to physical therapy practices across diverse patient populations.
• Experience in discharge planning, home & community care, outpatient clinics, or hospital environments.
• Proficiency with EMR (Electronic Medical Records) systems for accurate documentation and communication.
• Excellent patient care skills with a compassionate approach tailored to geriatrics, orthopedics, or other specialized areas.
• Ability to perform detailed assessments involving medical terminology understanding and clinical reasoning.

Pay: $75.00 - $175.00 per hour
