<Job Summary>
We are looking for a Physical Therapist to see patients at their homes in The Villages, FL. We are seeking a dedicated and experienced Home Health Physical Therapist to join our multidisciplinary team. The ideal candidate will possess a strong background in physiotherapy, with expertise in developing individualized care plans and delivering comprehensive patient care within home and community settings. This role offers the opportunity to make a meaningful impact on patients’ recovery and quality of life by providing expert physiotherapy services across diverse populations.

<Responsibilities>

• Assess patients’ physical conditions, including mobility, strength, and functional abilities, to develop tailored treatment plans.
• Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics to promote recovery and improve patient outcomes.
• Coordinate discharge planning with healthcare teams to ensure seamless transitions from inpatient or outpatient settings to home care.
• Deliver physiotherapy services across various environments, including home health, outpatient clinics, hospitals, and inpatient units.
• Educate patients and their families on exercises, mobility aids, and injury prevention strategies to support ongoing recovery.
• Maintain accurate documentation of patient progress using electronic medical record (EMR) systems while adhering to confidentiality standards.
• Collaborate with interdisciplinary teams to modify care plans based on patient progress and changing needs, especially in pediatrics and complex cases requiring specialized attention.

<Skills>

• Extensive knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
• Proven experience in discharge planning, acute care, inpatient, outpatient, and home health environments.
• Strong understanding of care plans for diverse populations including pediatrics, geriatrics, orthopedics, and patients with chronic conditions.
• Familiarity with EMR systems for documentation and patient record management.
• Excellent patient care skills with an emphasis on compassionate communication and education.
• Ability to apply advanced techniques such as trigger point therapy within a safe and effective treatment framework.
• Knowledge of home & community care practices combined with hospital experience enhances the ability to deliver comprehensive physiotherapy services. Pay: $44.49 - $53.58 per hour
