Duties

• Conduct comprehensive assessments of patients’ physical condition, including mobility, strength, balance, and functional abilities.
• Develop and implement individualized care plans that incorporate evidence-based physiotherapy techniques tailored to each patient’s needs.
• Deliver hands-on therapy sessions utilizing modalities such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient rehabilitation strategies.
• Coordinate discharge planning with healthcare providers to ensure smooth transitions from hospital or inpatient settings to home or community environments.
• Educate patients and their families on injury prevention, exercise routines, medication management, and safety precautions to promote long-term health.
• Maintain detailed documentation of patient progress using electronic medical record (EMR) systems in compliance with regulatory standards.
• Collaborate with interdisciplinary teams including physicians, nurses, social workers, and caregivers to optimize patient outcomes across all care settings.

Requirements

• Valid licensure as a Physical Therapist with current registration in the relevant jurisdiction.
• Proven experience in home health care environments, including outpatient clinics or hospital settings with home & community care exposure.
• Strong knowledge of physiology, anatomy, medical terminology, and therapeutic techniques such as trigger point therapy and orthopedics.
• Demonstrated ability to develop effective care plans for diverse populations including geriatrics and pediatrics.
• Experience with discharge planning processes and managing patient care across inpatient, outpatient, and home health settings.
• Proficiency in EMR systems for documentation and communication purposes.

Pay: $120,000.00 - $160,000.00 per year
