Pay Range: $75– $110 per visit (based on experience and case type)

Position Overview

We are seeking a motivated Physical Therapist to provide home-based care to patients with a variety of functional impairments. This role involves evaluating patients, developing individualized treatment plans, and delivering high-quality care aimed at maximizing independence and mobility. You’ll also collaborate with the broader care team and maintain accurate clinical documentation.

Key Responsibilities

• Perform initial evaluations and ongoing assessments to determine patient needs
• Develop and implement personalized treatment plans
• Deliver skilled therapy services focused on strength, mobility, and functional outcomes
• Complete documentation in a timely and compliant manner
• Communicate effectively with patients, families, and interdisciplinary team members
• Supervise PTAs and therapy aides when applicable
• Participate in care coordination and case discussions as needed

Qualifications

• Graduate of an accredited Physical Therapy program
• Active Michigan Physical Therapist license
• Home health experience preferred

Skills & Attributes

• Strong clinical decision-making and problem-solving ability
• Self-motivated and comfortable working independently
• Excellent communication and interpersonal skills
• Familiarity with EMR systems for efficient documentation and communication within healthcare teams. Kinnser/Wellsky experience highly preferred.
• Ability to safely assist patients with mobility and transfers (with appropriate equipment)

Job Types: PRN, Contract, Per diem

Pay: $75.00 - $110.00 per hour
