Phoenix, Chandler, Gilbert, Mesa, Tempe, and surrounding East Valley communities.

What You’ll Do

• Deliver skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT)
• Implement individualized treatment plans focused on improving mobility, strength, balance, endurance, and functional independence
• Accurately follow the established Plan of Care (POC) and report patient progress, changes, or concerns to the supervising PT
• Perform and document therapeutic exercises, gait training, transfer training, and use of assistive devices
• Educate patients and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques
• Complete timely, accurate, and compliant documentation in accordance with agency policy, Medicare guidelines, and regulatory standards
• Travel to patient homes within designated service area and manage daily scheduling efficiently
• Participate in case conferences, team meetings, and quality improvement initiatives as needed
• Maintain current licensure, certifications, and competency requirements
