Job Summary:

Homeside Rehab is seeking a highly motivated and experienced Physical Therapist or Occupational Therapist to join our team. As a valued member of our team, you will provide quality Physical and Occupational Therapies to patients in their homes, while collaborating with our clinical staff to develop treatment plans that meet the unique needs of each patient.

Key Responsibilities:

• Evaluate and develop a plan of care according to the patient's prior and current functionality

• Treat patients with therapeutic practices designed to relieve pain, increase mobility, restore normal function, and maintain maximum performance

• Maintain accurate and timely documentation of individualized care

• Collaborate with clinical staff to develop treatment plans that meet the needs of patients

• Adhere to industry standards and medical guidelines

Requirements:

• Active NYS PT or OT License (Required)

• Basic computer literacy skills

• 1 year Home Health, Outpatient, SNF experience preferred

• Good communication between office staff and therapists
