Role Overview:
As a Home Health Physical Therapist, you will deliver personalized therapy to patients in their homes, collaborate closely with families and caregivers, and contribute to patient recovery with your expertise. Your role is vital in helping patients achieve their therapy goals and enhance their quality of life.

Key Responsibilities:

• Provide physical therapy services according to the physician’s plan of care.
• Educate patients, families, and caregivers on therapy goals and techniques.
• Refer patients to additional home health services as necessary.
• Prepare detailed clinical progress summaries and contribute to discharge planning.
• Supervise physical therapy assistants and engage in collaborative peer consultations.

Qualifications:

• Valid physical therapy license.
• Minimum of 1 year of physical therapy experience (home health experience preferred).
• Excellent communication and organizational skills.
• Current CPR certification.
• Reliable vehicle with insurance.
