Home Health PRN Physical Therapist Assistant

Haven Home Health is seeking a PRN Physical Therapist Assistant (PTA) to join our home health team. In this role, you will work under the direction of a licensed Physical Therapist to provide therapy services that help patients improve mobility, independence, and quality of life in the comfort of their homes.

This position is ideal for a motivated PTA who enjoys working independently while collaborating with a supportive clinical team.

PRN (as needed)

Responsibilities

  • Provide physical therapy treatments under the direction of a licensed Physical Therapist

  • Assist patients with therapeutic exercises, mobility training, and functional activities

  • Educate and motivate patients to improve strength, mobility, and daily living skills

  • Observe patient responses to treatment and report progress to the supervising Physical Therapist

  • Assist patients with the proper use of assistive devices such as walkers, crutches, and wheelchairs

  • Maintain accurate and timely documentation in the patient’s medical record

  • Communicate effectively with the therapy team and other healthcare providers

  • Participate in care coordination to ensure patients receive quality home health services

  • Maintain patient confidentiality and comply with company policies and regulatory guidelines

  • Attend staff meetings or trainings as required

Qualifications

  • Current Oklahoma PTA license required

  • Associate degree from an accredited Physical Therapist Assistant program

  • Home health experience preferred

  • Experience in direct patient care preferred

  • Current CPR certification required

  • Valid driver’s license, reliable transportation, and auto liability insurance required

  • Strong communication and documentation skills

  • Ability to work independently in a home health environment

Work Environment

  • Frequent travel throughout the service area

  • Work in a variety of home environments

  • Flexible schedule based on patient needs

Physical Requirements

  • Ability to lift up to 50 lbs

  • Ability to assist with patient mobility and positioning

  • Ability to maintain focus during frequent interruptions

  • Ability to communicate clearly with patients and care teams
