Role Overview:
As a Home Health PTA, you will work directly with patients in their homes to implement therapy treatments, assist with rehabilitation exercises, and support their journey to improved mobility and quality of life. You will collaborate with Physical Therapists, families, and caregivers to ensure the best outcomes for our patients.

Key Responsibilities:

• Provide physical therapy treatments and assist with rehabilitation exercises according to the plan of care developed by the Physical Therapist.
• Educate patients, families, and caregivers on exercises, therapy techniques, and home modifications.
• Monitor patient progress, document outcomes, and report any changes or concerns to the Physical Therapist.
• Assist in the development and adjustment of patient care plans as needed.
• Ensure compliance with safety protocols and therapy standards.

Qualifications:

• Valid Physical Therapist Assistant (PTA) license.
• Minimum of 1 year of experience as a PTA (home health experience preferred).
• Strong communication and organizational skills.
• Current CPR certification.
• Reliable vehicle with insurance.
