Central Coast Home Health and Hospice is a locally owned organization with an excellent reputation throughout the San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking a Full time Home Health Referral Coordinator (Licensed vocational nurse, medical assistant or certified nurse assistant) to join our team in our San Luis Obispo office.
The Referral Coordinator is responsible for ensuring that patient care is coordinated and managed efficiently and effectively while working closely with the home health intake team. This position also verifies patient demographics, reviews orders and labs and communicating with our field clinicians to ensure that patient care and services are delivered appropriately.
The regular work schedule is 8 AM-5 PM, Monday-Friday, with rotating 1 weekend day every 4 wks.
