Job Summary
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders, agency policies, and applicable state and federal regulations. The RN is responsible for patient assessment, care planning, implementation of nursing interventions, patient and family education, and coordination of care to promote optimal health outcomes and independence.
Key Responsibilities Clinical Care
- Conduct comprehensive initial and ongoing patient assessments in the home setting
- Develop, implement, and update individualized plans of care based on patient needs and physician orders
- Administer medications, injections, wound care, IV therapy, catheter care, and other skilled nursing procedures as required
- Monitor patient condition and promptly report changes to the physician and care team
- Evaluate patient response to treatments and adjust care as needed
Patient & Family Education
- Educate patients and caregivers on disease processes, medications, treatments, and self-care techniques
- Promote patient safety, infection control, and fall prevention in the home
- Provide emotional support and counseling to patients and families
Care Coordination
- Collaborate with physicians, physical therapists, occupational therapists, social workers, and other healthcare professionals
- Participate in interdisciplinary case conferences and care planning
- Ensure continuity of care through effective communication and coordination
Documentation & Compliance
- Accurately document assessments, interventions, patient progress, and outcomes in a timely manner
- Maintain compliance with Medicare, Medicaid, and other regulatory standards
- Follow agency policies, procedures, and infection control guidelines
Professional Practice
- Maintain current nursing licensure and required certifications
- Adhere to ethical and professional nursing standards
- Participate in quality improvement activities and continuing education
Qualifications Required
- Current Registered Nurse (RN) license in good standing
- Graduate of an accredited nursing program
- Valid driver’s license and reliable transportation
- Current CPR/BLS certification
- Ability to work independently and manage time effectively
Preferred
- Minimum of one year of clinical nursing experience
- Home health or community nursing experience
- Experience with electronic health records (EHR)
- Knowledge of Medicare home health regulations
