<Job Summary>
We are seeking a dedicated and skilled Home Health Registered Nurse to deliver exceptional nursing care within the home setting. The ideal candidate will possess a strong clinical background, excellent assessment skills, and a passion for improving patient outcomes. As a vital member of our healthcare team, you will coordinate and implement personalized care plans, ensuring safety, comfort, and well-being for our patients.
<Responsibilities>
- Conduct thorough patient assessments including vital signs, physical examinations, and review of medical histories to determine individual needs.
- Develop and implement comprehensive care plans tailored to each patient's condition, including medication administration and IV infusion therapies.
- Provide skilled nursing interventions such as wound care, suturing, ostomy care, catheterization, and aseptic techniques.
- Educate patients and their families on disease management, medication usage, and health coaching to promote independence and health literacy.
- Monitor patient progress through ongoing evaluation and documentation using electronic health record (EHR) systems like Epic or Cerner.
- Collaborate with physicians and interdisciplinary teams to coordinate care across settings including hospital medicine, ICU, trauma centers, or outpatient clinics.
- Ensure compliance with HIPAA regulations and maintain accurate medical records in accordance with NCQA standards.
- Participate in discharge planning and case management activities to facilitate smooth transitions between care environments.
<Skills>
- Extensive clinical experience with home care nursing practices including IV insertion, tube feeding, ventilator management, and infection control protocols.
- Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for documentation and patient data management.
- Strong knowledge of physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.
- Experience working with diverse patient populations including geriatrics, pediatrics (including neonatal ventilator care), individuals with developmental disabilities or dementia care needs.
- Familiarity with specialty areas such as hospice & palliative medicine, behavioral health, acute pain management, trauma medicine (Level I & II trauma centers), infectious disease care, occupational health, and long-term care settings like nursing homes or assisted living facilities.
- Ability to perform physical examinations accurately and utilize aseptic techniques during procedures such as phlebotomy or sterile processing.
- Excellent communication skills for triage assessments, patient education sessions, and interdisciplinary collaboration.
