Home Health Registered Nurse

Overview
We are seeking a dedicated and compassionate Registered Nurse to join our Home healthcare team. The ideal candidate will possess a strong commitment to patient care and be skilled in various nursing practices. This role requires the ability to work collaboratively with healthcare professionals and provide high-quality care to patients across different settings, including hospitals, long-term care facilities, and outpatient clinics.

Duties
As a Home Health Registered Nurse, your responsibilities will include but are not limited to:

Conducting comprehensive patient assessments and developing individualized care plans.
Administering medications and treatments as prescribed.
Monitoring vital signs and managing acute pain for patients in various settings including critical care and emergency medicine.
Utilizing electronic health record (EHR) systems for medical documentation.
Collaborating with interdisciplinary teams to ensure optimal patient outcomes.
Educating patients and families on health management, medication administration, and post-discharge care.
Maintaining compliance with HIPAA regulations and NCQA standards while ensuring infection control measures are followed.
Engaging in case management activities for patients with complex needs.

Experience
Candidates should possess the following qualifications:

Current Registered Nurse license with relevant home health care experience for a minimum of 2 years with at least 1 year in a supervisory position.
Familiarity with medical terminology, anatomy knowledge, physiology knowledge, and diagnostic evaluation processes.
Experience working with diverse populations including individuals with developmental disabilities or those requiring memory care.
Proficiency in using EMR systems for patient documentation and data management.
Knowledge of ICD coding (ICD-9/ICD-10) and CPT coding is a plus.
