Overview
We are seeking a dedicated and compassionate Registered Nurse to join our Home healthcare team. The ideal candidate will possess a strong commitment to patient care and be skilled in various nursing practices. This role requires the ability to work collaboratively with healthcare professionals and provide high-quality care to patients across different settings, including hospitals, long-term care facilities, and outpatient clinics.

Duties
As a Home Health Registered Nurse, your responsibilities will include but are not limited to:

  • Conducting comprehensive patient assessments and developing individualized care plans.
  • Administering medications and treatments as prescribed.
  • Monitoring vital signs and managing acute pain for patients in various settings including critical care and emergency medicine.
  • Utilizing electronic health record (EHR) systems for medical documentation.
  • Collaborating with interdisciplinary teams to ensure optimal patient outcomes.
  • Educating patients and families on health management, medication administration, and post-discharge care.
  • Maintaining compliance with HIPAA regulations and NCQA standards while ensuring infection control measures are followed.
  • Engaging in case management activities for patients with complex needs.

Experience
Candidates should possess the following qualifications:

  • Current Registered Nurse license with relevant home health care experience for a minimum of 2 years with at least 1 year in a supervisory position.
  • Familiarity with medical terminology, anatomy knowledge, physiology knowledge, and diagnostic evaluation processes.
  • Experience working with diverse populations including individuals with developmental disabilities or those requiring memory care.
  • Proficiency in using EMR systems for patient documentation and data management.
  • Knowledge of ICD coding (ICD-9/ICD-10) and CPT coding is a plus.

Pay: $1,500.00 - $1,750.00 per week
