Duties
- Conduct thorough patient assessments including vital signs, medical history, and current health status to determine individualized care needs.
- Develop and implement comprehensive care plans that align with clinical guidelines and patient goals, ensuring optimal health outcomes.
- Coordinate post-acute care transitions, discharge planning, and outpatient services to facilitate smooth patient recovery pathways.
- Monitor patient progress through regular visits and telehealth check-ins, adjusting care plans as necessary based on clinical findings.
- Collaborate closely with physicians, therapists, social workers, and other healthcare professionals to deliver integrated patient care.
- Manage documentation of medical records, treatment plans, and progress notes in compliance with HIPAA regulations.

Qualifications
- Valid Registered Nurse (RN) license with current state licensure.
- Critical care experience preferred; background in ICU, emergency medicine, or Level I Trauma Center highly desirable.
- Experience in hospital medicine or post-acute settings such as skilled nursing or outpatient clinics.
- Knowledge of physiology, medical terminology, diagnostic evaluation, and utilization management principles.
- Strong skills in patient assessment, discharge planning, case management, and medical records documentation.
- Familiarity with HIPAA regulations and confidentiality standards for protected health information.
- Ability to perform comprehensive patient evaluations including vital signs and clinical observations across diverse populations such as pediatrics, geriatrics, or behavioral health. Pay: $63,000.00 - $83,000.00 per year
