Pinnacle Home Health is hiring Registered Nurses (RN). Full-time, part-time, and per-diem options are available. Home health nursing is a great opportunity for independent clinicians, which allows flexibility in work schedule.
Job Requirements:
Key Responsibilities, Admissions & Patient Care
- Initiate and develop individualized plans of care
- Provide skilled nursing care in accordance with the nursing process and state regulations
- Administer medications and treatments as ordered
- Educate patients and families on Home health services, care plans, and symptom management
- Support patients with care and activities of daily living as needed
Care Coordination & Communication
- Communicate effectively with physicians to obtain orders and report patient status
- Coordinate care with the clinical team and community resources
- Maintain accurate, timely clinical documentation
- Provide education and guidance to patients, families, and caregivers
Additional Responsibilities
- Assist with coordination of equipment, supplies, and services needed for admission
- Support continuity of care
- Supervise and delegate tasks to Clinical team when appropriate
- Engage in ongoing professional development and training
- Participate in quality improvement initiatives
Qualifications
- Graduate of an accredited nursing program
- Current California RN license and CPR certification
- Minimum of 1 year of RN experience in (hospice &home health preferred and Kinser knowledge
- Strong assessment, communication, and organizational skills
- Ability to work independently with minimal supervision
- Valid driver’s license and reliable, insured vehicle

Compensation for this role is per-visit. Actual earnings depend on visit type (SOC, ROC, Recert, Revisit, Discharge) and caseload mix. Competitive per-visit rates are discussed during the interview process.
