Primary Work Location:
TLC: This program delivers skilled home health services in non-traditional settings. Through a management agreement, Cardea Health operates TLC Homecare Inc., providing nursing and physical/occupational therapy directly to individuals in shelters, encampments, and other non-traditional environments. Services are designed to meet patients where they are, reducing barriers to care while supporting health, stability, and dignity. Eligibility is based on referral for skilled home health services, regardless of housing status, behavioral health conditions, or social instability.
Job Summary:
Cardea Health is a nonprofit healthcare organization that provides clinical services to vulnerable populations. We currently have an opening for a home health registered nurse. Responsibilities: Choose to be an integral part of a team that focuses on care of patients in their home. This may include providing care to individuals in unstable and/or non-traditional housing environments, such as shelters or unsheltered environments. The registered nurse will be responsible for both evaluation and treatment of their patient caseload, and supervision of licensed vocational nurses. Prior home health experience is preferred but not required.
Responsibilities:
Perform initial and ongoing clinical assessments to identify patient needs, determine skilled needs, and develop plan of care
Provide skilled care as ordered and ongoing evaluation of client needs
Evaluate outcomes of care with appropriate discharge planning.
Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Accept clinical assignments that are consistent with education and competence to care for patients.
Complete appropriate documentation in a timely manner.
Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs.
Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
Promote company philosophy and administrative policies.
Minimum Required Skills & Qualifications:
Education: Bachelor or associates degree in nursing from a BRN accredited school
Licensure: Current California State License as a registered nurse, current California Driver’s License
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR. Knowledge of home care needs among vulnerable populations
Transportation: Reliable transportation and valid and current auto liability insurance
Preferred Skills & Qualifications:
Experience: 1+ years of Home Health nursing experience (preferred)
Physical Requirements:
Occasional sitting or stationary work
Frequent standing, walking, kneeling, stooping, and ascending stairs
Occasional lifting and carrying up to 20 lbs
Frequent pushing and pulling
Frequent reaching high, low, and level
