Home Health Review Nurse - RN Required
As a Home Health Review Nurse (RN) you will play a critical role in ensuring quality, accuracy, and compliance in patient care documentation. As a member of our home health team, you’ll support clinical staff and processes that promote continuity of care, regulatory compliance, and optimal patient outcomes.

As a Home Health Review Nurse (RN), you will:
- Review and audit clinical documentation, including OASIS assessments, for accuracy, completeness, and timeliness.
- Ensure all patient records and coding practices meet regulatory and compliance standards.
- Process physician orders and Plans of Care (POC) to ensure timely communication and approval.
- Collaborate with clinical staff to clarify documentation and resolve discrepancies.
- Monitor outsourced coding practices to ensure accuracy and timeliness.
- Assist with insurance reviews, ADRs (Additional Documentation Requests), and appeals as needed.
- Review patient episodes from admission through discharge to ensure continuity and quality of care.
- Coordinate transfer documentation and communication with facilities for patient transitions.
- Export OASIS assessments within required timeframes.
To qualify for a Home Health Review Nurse (RN) with us:
- Education: Associate’s Degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Current license to practice as a Registered Nurse (RN) in Indiana.
- Previous review or audit experience required.
- Prior home health or hospice experience preferred.
- Strong attention to detail, organizational, and communication skills.
- Proficiency in computer documentation systems.
