Registered Nurse (RN) – Home Health Field Nurse
Coverage Area: Santa Monica, Malibu, Marina del Rey, Culver City, Pacific Palisades, Beverly Hills**

We are seeking an experienced Registered Nurse (RN) for a Home Health field position covering Santa Monica, Malibu, Marina del Rey, Culver City, Pacific Palisades, and Beverly Hills.

DO NOT APPLY if you do not have recent Home Health experience and OASIS documentation experience.
Experience will be verified.

MANDATORY REQUIREMENTS (No Exceptions):

• Active California RN license in good standing
• Minimum 2 year recent Home Health experience
• Strong OASIS documentation experience
• Proficiency in AXXESS software
• Ability to complete OASIS documentation within 3 days of Start of Care (SOC)
• Current BLS certification
• All vaccinations up to date
• Current TB test and physical
• Valid driver’s license and reliable transportation
• All credentials and compliance documents current

Clinical Experience Must Include:

• Start of Care (SOC) assessments
• OASIS documentation and submission
• Wound care (including complex wounds)
• Post-surgical patient management
• Medication reconciliation and education
• Care plan development and updates
• Skilled nursing follow-up visits

Position Responsibilities:

• Perform Start of Care (SOC) visits
• Complete and submit OASIS documentation within 3 days of SOC
• Conduct skilled nursing follow-up visits as needed
• Coordinate care with physicians and interdisciplinary team
• Provide patient and family education
• Complete accurate and timely documentation in AXXESS
• Work independently in the field

This is a fast-paced field position requiring strong clinical judgment, time management, and documentation skills.

Only experienced Home Health RNs meeting ALL requirements will be considered.

Pay: $80.00 - $180.00 per hour
