Home Health RN in East County Weekender

Job Summary
We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to provide exceptional patient care in East County. This vital role involves delivering personalized nursing services directly to patients in their homes, ensuring their health, safety, and comfort. As a Home Health RN, you will coordinate care plans, perform comprehensive assessments, and collaborate with families and healthcare teams to promote optimal health outcomes. Your expertise will help empower individuals to maintain independence and improve their quality of life through skilled nursing interventions.

Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews.
  • Develop and implement individualized care plans aligned with physician orders and patient needs.
  • Administer medications via IV infusion, injections, and other routes while adhering to aseptic techniques and safety protocols.
  • Perform wound care, suturing, ostomy care, and catheterization with precision and compassion.
  • Manage complex conditions such as acute pain, infectious diseases, dementia, Alzheimer's care, and memory care.
  • Educate patients and families on disease management, medication administration, nutrition, and health coaching strategies.
  • Document all patient interactions accurately within EMR (Electronic Medical Records) systems like Epic or Cerner while maintaining HIPAA compliance.

Qualifications

  • Valid RN license in <LOCATION> with current CPR certification (Basic Life Support).
  • Proven experience in home care or medical-surgical nursing; prior long-term care or hospice experience is highly valued.
  • Strong knowledge of physiology, anatomy, ICD coding (International Classification of Diseases), ICD-10/ICD-9 coding standards, and medical terminology.
  • Experience working with individuals with disabilities or developmental disabilities; familiarity with geriatrics and behavioral health preferred.
  • Ability to perform complex procedures such as IV insertion, phlebotomy, laboratory specimen collection/processing, and ventilator management if applicable.
  • Proficiency with EMR/EHR systems like Athenahealth or eClinicalWorks; excellent patient assessment skills.
  • Critical care experience including ICU or Level I/II trauma center background is advantageous.
