Home Health Scheduling Coordinator

Occupation Summary

The Home Health Scheduling Coordinator is responsible for coordinating clinician schedules to ensure timely delivery of patient care services while maximizing staff productivity, maintaining continuity of care, and supporting operational efficiency. This role serves as a central communication hub between clinicians, patients, referral sources, and leadership to ensure visits and starts of care are scheduled accurately and in compliance with agency standards and regulatory requirements.

Duties and Responsibilities

· Schedule start of care home health visits for nursing and therapy according to physician orders and patient care plans.

· Coordinate daily and weekly clinician schedules to optimize productivity and geographic efficiency.

· Ensure timely scheduling of admissions, resumptions of care, recertifications, and routine and supervisory visits.

· Monitor visit frequencies and authorization requirements to ensure compliance with orders and payer guidelines.

· Adjust schedules in real time to accommodate patient needs, call-outs, and urgent requests.

· Maintain accurate clinician availability, PAL, and on-call schedules.

· Assist with coverage planning and reassignment of visits as needed.

· Support clinicians in resolving scheduling conflicts or barriers to patient care.

· Serve as a primary point of contact for patients regarding visit scheduling.

· Maintain strong customer service standards when interacting with patients and families.

· Maintain accurate scheduling documentation within the EMR system.

· Track and monitor unassigned visits and scheduling gaps.

· Collaborate with Admissions, Clinical Managers, and Billing teams to ensure seamless patient care coordination.

· Verify patient demographics, visit frequencies, and service needs prior to scheduling.

· Support clinician productivity and assist leadership with productivity tracking and scheduling reports.

· Maintain minimal unassigned visits.

· Ensure visits are scheduled within ordered frequency timelines.

· Support clinician productivity and efficient route planning.

· Demonstrate timely communication and problem resolution.

· Maintain high accuracy in scheduling documentation.

· Perform other duties as assigned.

Qualifications

· Minimum of one year of healthcare scheduling experience.

· Knowledge of home health operations, disciplines, and visit frequencies strongly preferred.

· Experience in customer service and interpersonal relationships preferred

· Proficient in all Microsoft Office software

· Strong verbal and written communication skills

· Ability to work in a fast-paced environment, meet deadlines, and work independently with minimal supervision

· Exceptional attention to detail and accuracy in documentation

· Valid state driver's license and satisfactory motor vehicle driving record per agency and insurance provider standards; proof of auto insurance

· Ability to lift 25 lbs.
