Job Overview
Ohman Family Living is hiring a Speech Language Pathologist SLP to provide in home speech therapy services across Northeast Ohio. This role focuses on helping patients improve communication, cognition, and swallowing function in a home health setting.

This is a great opportunity for an SLP looking for flexible PRN work with autonomy and meaningful one on one patient care.

Key Responsibilities

• Perform speech therapy evaluations in patient homes
• Develop and implement individualized treatment plans
• Treat speech, language, cognitive, and communication disorders
• Assess and treat swallowing disorders and dysphagia
• Educate patients and caregivers on exercises and care plans
• Recommend strategies to improve safety and function at home
• Collaborate with RN, PT, OT, and interdisciplinary care team
• Complete accurate and timely EMR documentation
• Follow Medicare and home health regulations

Qualifications

• Licensed Speech Language Pathologist SLP in Ohio
• Master’s degree in Speech Language Pathology
• CCC SLP preferred
• Home health or rehab experience preferred
• Experience with adult and geriatric patients preferred
• Valid driver’s license and reliable transportation
• Strong communication and documentation skills
• Comfortable working independently in patient homes
