Homecare Intake Coordinator

PARC is looking for an organized, empathetic Intake Coordinator to serve as the vital first point of contact for new clients and healthcare partners.

JOB SUMMARY

The Intake Coordinator coordinates with referral sources and Agency staff to assure adequate information is obtained. The Intake Coordinator will process and transcribe home care referrals and enter date into EMR system.

QUALIFICATIONS

  • Minimum 1–2 years of healthcare administrative experience; home health, or hospice experience strongly preferred.
  • Experience with EMR system
  • High school diploma or GED required; Associate or Bachelor’s degree in healthcare management or social work preferred.

RESPONSIBILITIES

  • Answer incoming calls timely, with a professional tone and routing calls as necessary
  • Assisting with a variety of administrative task including copying, faxing, taking notes, creating folder for varies departments
  • Make Calls to Patients/Hospitals/Doctors’ Offices
  • Provide excellent customer service
  • Compliant with accepted professional standards and practices.
  • Ability to read and interpret clinical documents, such as discharge plans, physician progress notes, and orders.
  • Avid team player.
  • Consistently maintains a positive attitude which promotes team and optimal performance.
  • Process referrals and initial communication with patient and/or patient representative for home care admission or resumption.
  • Serve as a resource to case managers and physicians concerning home health, availability, and admission procedures.
  • Adhere to the policies and procedures related to referral, intake, and data entry.
  • Assist in all administrative areas of the department, as needed, including insurance verification, and obtaining authorization, as needed.
  • Maintain departmental computerized tracking for specific database reporting.
  • Provide education to referral sources and Agency staff related to necessary items in processing referrals.

Job Type: Full-time

Pay: $18.00 - $23.00 per hour
